New Patients
If you live within our practice area
and would like to register as a patient, please come to the surgery.
Please note that we only register new patients between 10am and 4pm. We register new patients outside these hours by appointment only.
You will need to complete a registration form and a patient health
questionnaire. If you wish to register your children you will need
to complete a registration form and questionnaire for each child.
To speed up the process, the patient health questionnaire can be
downloaded by clicking on the links below and completed prior to
attending the surgery.

We will need to see a form of photo ID, for example your passport,
driving licence or a bus pass. We will also need to see a proof of
address which must have your name and address on it. Examples of a
suitable proof of address are a utility bill or a bank statement. We regret we are unable to accept mobile phone bills as proof of address
Once you are registered you can access all of our services.
