New Patient Registration

If you live within our practice area and would like to register as a patient, please come to the surgery, or fill in the registration forms (see below for forms) and send them to our email address.

You will need to collect and complete a registration form from the Surgery together with a patient health questionnaire. If you wish to register your children you will need to complete a registration form and questionnaire for each child.

We will need to see a form of photo ID, for example your passport, driving licence or a bus pass. We will also need to see a proof of address which must have your name and address on it. Examples of a suitable proof of address are a utility bill or a bank statement. We regret we are unable to accept mobile phone bills as proof of address.  You will be required to sign a consent regarding your Data and how you require us to use it.

Please ensure you complete the GMS1 form and then the appropriate registration form for each family member.

To be able to access appointments for results you will need online access, please use the links below. Forms will not be accepted if not signed and sent in with the correct ID.

Online Access - Proxy

Proxy access form needs to be completed if you would like someone else to have access to your records or you would like to access someone elses records. If this is for a child over the age of 11 they will need to sign the form to confirm they are happy for you to have access to this. At 16 your access will be stopped automatically and will need to be re confirmed that the child is still happy for you to have access to their records.
